The most basic medical procedure and the one which most people are used
to accessing on a regular basis, is a simple visit to a GP (general
practitioner) followed by a diagnosis and the prescribing of medication.
In the vast majority of cases, the patient takes this prescription,
usually something as simple as an anti-biotic, painkillers or other
basic medication, along to a pharmacy and has it made up for them.
Occasionally, however, something goes wrong, and when it does, the
effects can be fairly drastic.
There are cases where the wrong
drug is prescribed based on faulty diagnosis or the pharmacist gives out
the wrong medication for one reason or the other or the lab scientist
gives a wrong test result or the nurse makes a mistake in her work.
Some
other cases that we've heard of are cases of errors in surgery (like
forgetting a surgical tool inside the patient!). Now that's quite
extreme.
Let's not forget, these people are human hence can make mistakes. However, too many mistakes is not acceptable.
